Chronic Conditions

Are you having a hard time dealing with your chronic condition or caring for a loved one with a chronic condition? These programs are offered to you by UT Extension Sevier County and the Sevier County Health Department. We want you to adjust easily to your diagnosis and learn skills to help with live with your chronic condition.

We do not offer medical advice during any of this program. We offer these in-person or Zoom programs to help support you as you work with a medical professional.

Upcoming Programs:

A six-week evidence-based program that helps individuals take day-to-day responsibility for their diabetes care.

Participants gain skills necessary to self-manage their diabetes and work effectively with their healthcare professionals.

Dates: December 2nd, 5th, 9th, 12th, 16th, & 19th 2024

The goal of the program is the reduction of breast and cervical cancer mortality through increased awareness, early detection, diagnosis, and treatment.


Program Details:

The Sevier County Health Department will be doing a Breast Cancer Awareness Walk on October 12th at the Seven Islands Birding Park (2809 Kelly Lane, Kodak).

A six-week evidence-based group education program that teaches people practical skills they need to live well with a variety of health conditions.

Through weekly sessions, the workshop provides support for continuing normal daily activities and dealing with emotions that arise from having a chronic condition.
